In-The-Moment Coaching Helps Reduce Type 2 Diabetes Risk 

Yes Health’s mobile app provides in-the-moment coaching to help people with prediabetes adjust their eating habits, physical activity and foster well-being activities. These lifestyle changes can facilitate weight loss and lower your risk of developing Type 2 diabetes.

 

 
 

How do I participate in this one year pilot program?

  1. If you have been diagnosed as having prediabetes, ask your healthcare provider if they are offering the pilot program, as only select Primary Care Networks in Alberta are participating. There is no cost to you for the program and it is available to you for one year.

     

  2. Once enrolled in the program, you will be contacted by Yes Health coaches who have backgrounds in nutrition, fitness and well-being to provide support and education.

     

  3. You can take photos of your meals and get in-the-moment feedback on your food choices. You can also track your activity level, get timely feedback from experts, and access a library of fitness activities, recipes and well-being resources .
